If you are at high risk for developing wet AMD, you can now take a proactive approach to your eye health.
ForeseeHome AMD Monitoring Program is intended for use in the detection and characterization of central and paracentral metamorphopsia (visual distortion) in patients with AMD. It acts as an aid in monitoring the progression of disease factors. With early detection, treatment is more possible for AMD and better protects good, functional vision. This monitoring program is home based and meant to be use in conjunction with with routine eye exams at your doctor’s office. Dr. Rapkin is the only Indiana ophthalmologist in Indiana that offers this program. Foresee can often detect small changes in vision before you notice any symptoms yourself. With this program, you can test your eyes daily with the test only taking 3 minutes per eye. The results are automatically transmitted to the ForeseeHome Data Monitoring Center through a built-in cellular modem or phone line. The test results are compared to a normative database and to your personal baseline. If a statistically signifiant change in your test results is detected, Dr. Rapkin will be notified so he can schedule an appoint with you.

Yes. The ForeseeHome Program provides ongoing monitoring between your regular office visits. There may be instances where changes in your test results trigger an alert but the doctor does not diagnose wet AMD or where your test results do not trigger an alert but your doctor diagnoses wet AMD.
No. Diagnosis is the responsibility of the prescribing eye doctor. The ForeseeHome provides functional data about your vision which your doctor can incorporate into a comprehensive patient assessment.
No, the ForeseeHome does not require a home computer or internet connection. To start using your device, simply plug it into a power outlet.
No. Each device is personally calibrated to a single individual.
Some variability in testing results is typical. The ForeseeHome is designed to trigger an alert when test result changes are considered statistically significant. In the event of an alert, the Notal Vision Data Monitoring Center will contact your eye doctor so an appointment can be scheduled as soon as possible.
Yes. Test reports can be viewed at any time by you and your doctor through the secure online database. (To gain access to your reports, we will need a signed waiver from your physician.)
Patients must have a confirmed diagnosis of dry AMD, no existing condition which could compromise test results, visual acuity of 20/60 or better, and the ability to use the ForeseeHome device.
The lightweight monitor (in its original packaging) was designed for easy travel. In addition, you can send data from any location and no long distance service is required. If no phone line or cellular service is available, you can still test daily. The device will store all data and then transmit it once connected back into a phone or cellular service is available.
