Q: Will my insurance reimburse for this product?

A: Reimbursement is currently not available from Medicare or private insurance. We hope to have reimbursement in the future.

Q: How often should I take the ForeseeHome test?

A: Once a day is the standard recommendation.

Q: Do I still see my eye doctor for my regular visits?

A: Yes. The ForeseeHome Program provides ongoing monitoring between your regular office visits.  There may be instances where changes in your test results trigger an alert but the doctor does not diagnose wet AMD or where your test results do not trigger an alert but your doctor diagnoses wet AMD.

Q: Does the ForeseeHome diagnose wet AMD?

A: No.  Diagnosis is the responsibility of the prescribing eye doctor. The ForeseeHome provides functional data about your vision which your doctor can incorporate into a comprehensive patient assessment.

Q: Do I need a computer or internet to use the ForeseeHome device?

A: No, the ForeseeHome does not require a home computer or internet connection. To start using your device, simply plug it into a power outlet.

Q: Can more than one person use a single device?

A: No. Each device is personally calibrated to a single individual.

Q: What happens if a change in my results is detected?

A: Some variability in testing results is typical.  The ForeseeHome is designed to trigger an alert when test result changes are considered statistically significant.  In the event of an alert, the Notal Vision Data Monitoring Center will contact your eye doctor so an appointment can be scheduled as soon as possible.

Q: Can I see my test results?

A:  Yes. Test reports can be viewed at any time by you and your doctor through the secure online database.  (To gain access to your reports, we will need a signed waiver from your physician.)

Q: What are the eligibility criteria for enrollment in the ForeseeHome AMD Monitoring Program?

A: Patients must have a confirmed diagnosis of dry AMD, no existing condition which could compromise test results, visual acuity of 20/60 or better, and the ability to use the ForeseeHome device.

Q: How do I test if I go on vacation?

A: The lightweight monitor (in its original packaging) was designed for easy travel. In addition, you can send data from any location and no long distance service is required. If no phone line or cellular service is available, you can still test daily. The device will store all data and then transmit it once connected back into a phone or cellular service is available.

Q: Who do I contact if I have questions about the ForeseeHome?

A: Call the Notal Vision Customer Service Center at 888-910-2020 with any questions. Please do not call your doctor’s office.

What to Expect on Your First Visit
The patient procedure concerning paperwork and insurance is the same as for your normal eye doctor. At Retina Consultants, the patient’s pupil will be dilated and therefore you will need to arrange for a driver. This is true for all patients at all visits.

If you are at high risk for developing wet AMD, you can now take a proactive approach to your eye health. Frequent monitoring with the ForeseeHome™ AMD Monitoring Program provides a safety net for monitoring changes in your vision between office visits, allowing your doctor to detect and diagnose wet AMD at its earliest stage.
